Fail a story mission and leave the "Game Over" screen on. The music shoud slowly change into a some very strange sounds (like slowed down speech), followed by a woman's voice saying "Is, is that too loud?". It certainly creeped me out.

First go to NeoTokyo. Continue the mission normally. When you get hurt go back to where you get your weapons... in the alley. Go down the stairs at the end of the alley and turn right. Continue to the end of this new alley.
